Psychiatry in the Lone Star state is experiencing many of the same challenges as other branches of medicine
Uneven Supply
In the Lone Star state there are 2484 psychiatry doctors practicing today. Ninety-one of those physicians are D.O. trained, the vast majority being M.D. trained. Becoming a psychiatrist in the state of Texas is no more or less time intensive than most other states. It requires a four year undergraduate degree, a graduate degree in medicine (usually four years), and the successful completion of a psychiatry residency program which usually lasts around four years as well. These up and coming psychiatrists, as well as those practicing today, are faced with many challenges that are also facing the entire medical community as a whole. Among these are an uncertain employment climate, a weakening economy, and healthcare reform in addition to the ever-changing face of the field.
Psychiatry Specialty Overview/Introduction-
With its origins in the early 1800's, Psychiatry has developed and progressed differently than many other medical specialties. Research concludes that the psychiatric field may vary geographically. Southeastern Asian cultures, in particular, are prone to two geographically specific conditions called "amok" and "koro". Many other areas experience the same geographically specific phenomena. Psychiatrists from continent to continent, country to country, even state to state, face a different range of disorders and abnormalities affected by the particular socioeconomic populations they serve.
Psychiatry shortage/surplus-
In the United States today, we have approximately 32,200 practicing Psychiatrists. The median salary for the field is $176,520. The apparent decline of mental health and increased sense of hypochondria in the United States, along with PTSD, which I will discuss in greater detail later in the article are going to strengthen the demand for Psychiatrists, offering new jobs and more specialization, naturally. Also, with the expanding population, the medical field will also expand, and Psychiatry is expected to grow more than any other specialty. The outlook is very positive.
Weak Economy-
With home foreclosures, job losses, the stock market crash, and an overall weakened economy, mental health is on the decline. Suicides and homicides in over 25 European countries have increased due to economic hardships and a decreasing quality of life, and we can expect the same here. The need for Psychiatrists, therapists, and counselors will assuredly increase because of these hard times.
Post traumatic stress disorder Iraq and Afghanistan-
One of the major current issues facing the Psychiatric field today is Post Traumatic Stress Disorder. This condition has existed since the birth of conflict. The famed hero of legend, Achilles, was even said to experience this debilitating disorder when Patroclus was killed. Oddly, from era to era, even conflict to conflict, the symptoms of Post Traumatic Stress Disorder vary. Civil War veterans became easily irritated, experienced an inability to catch their breath, difficulty sleeping, and mood disorders. The "War to End All Wars", WWI, saw paralysis, shaking, and a drastic decrease of sight and hearing.
So what faces the psychiatric community today? Three years ago, over two hundred thousand veterans were receiving benefits for being disabled due to Post Traumatic Stress Disorder, totaling in a government expense of over four billion dollars.
Psychotherapy added to education about the disorder is one popular treatment combination, as well as counseling for the patient, couples, and family. Treating the sleep abnormalities, such as vivid nightmares, in these patients has shown highly effective at lessening the waking symptoms. Medication is also an option, and there are many medications that have shown helpful in recovery.
Health Insurance Reform-
One uncertainty, however, is the outcome of national health reform. If the reform actually works in increasing the enrollment into health care insurance programs, then when combined with the long term aging of the population, and these increased numbers, it should push demand for services and therefore cost higher. However, no one yet knows what will happen to reimbursement rates from Medicare and this new program. The question is will there be rate a cut of reimbursement rates for different procedures by Medicare and this new program that may conversely become a decreasing factor of the income of all specialties, including Psychiatry.
